Registration Form
Our service is for the Solihull area only.
If you are a carer aged 18 or over, either you or the person you care for must live in Solihull. If you are a young carer aged 5 – 17, then you or the person you care for must live in Solihull or, if you live out of borough, you must go to school in Solihull.
Please complete this form and provide as much information as possible about your caring role to support your registration. This will help us better understand your needs as a carer and ensure appropriate support can be identified.
By completing this registration, you consent—as a service provider working with Solihull Council—to the sharing of minimal information required for Solihull Metropolitan Borough Council (SMBC) to identify you. Any assessments and support plans we complete will also be shared with SMBC to ensure their records are up to date and to help identify services or benefits you may be entitled to or interested in.
If you’re referring a young or young adult carer, you can download a Microsoft Word version by clicking here.
